  Penis Trouble in 2 year old

I posted this on the circumcision thread but no one responded and I'm desperate. 
 
My third child was hospitalized for week beginning on the same night he was circumcised (for an unrelated issue) and part of his treatment included a catheter.  Since then we noticed that his penis is not quite the same as dad and big bro.  Either it became adhered or was never fully circumcised - I don't know because of the unusual circumstance that followed his circumcision. 
Anyway, he's now 2 years old (and healthy - Praise the Lord!) and often wakes up erect because he needs to use the bathroom.  This morning in particular he seemed very desperate and began to cry that his penis hurt.  Upon examination I saw that the skin had started to retract and was bleeding slightly.  I gave him a warm soapy bath and put on A&D Ointment for the night so that it wouldn't be uncomfortable while he slept.  Any other suggestions?  I hate doctors and he doesn't want anyone poking around with his sore parts, but do  you think I should call the pediatrician?  Thanks!     ~L

Oh, I also need to fin out where I can get information on caring for an intact penis.  (For a different son, of course!)

I can help you with the intact penis thing...both of my sons are intact and we do...nothing. Absolutely nothing. When they are older we'll teach them to retract it and clean it but basically, we bathe the kids and we don't do anything special down there and they have absolutely no problems at all. I have heard that sometimes there can be pain when the foreskin naturally seperates from the penis but so far I haven't had to deal with that. I would think that you took a good course when you gave him the warm bath.

My heart hurts for you and your little one. It is so hard to know what to do sometimes, so many choices.  He could have a UT infection?  I am the same way as you ...I have difficulty with docs sometimes.  I probably wouldn't mess around with something like this. I probably would find a doc to talk to. Some other encouragment when it comes to docs, interview and find a doc that supports many of your view points. I have been through many doctors and have one now I feel very comfortable with and would pay extra to see.  I think it's helpful to remember we're hiring them for a service, and it pays to find one that has similiar view points, if possible.  I will pray for you to have wisdom.  I am sure there some natural remedies that may help, but I am not knowledgable enough, yet...hope to be someday.

My son's circ began to reheal or wasn't pulled back enough in the beginning and I had to pull back and the doc did once as well.  There was some pain and slight tearing and one time even a dab of blood - which totally made me feel like an ogre, but it has since healed nicely and although he has more skin than Dad, there is no trouble retracting whenever necessary. 

Also, it's pretty normal for boys to become erect when the need to pee. 

My friend has 4 boys and she says that it's normal at a certain point for them to become erect more often and when this happens the circ will continue to pull back more and more on it's own.  Mine isn't this old yet, but this is what she told me.  Wink

The bath was probably good for soothing him at the same time as cleaning him.
 I would flush his system and treat him as if he had a U.T.I. even if he does not it won't hurt him.

OK, most likely I will go against the grain here....but I would bring him to the Doctor. Why risk the chance of it getting infected, plus if it was the circumcision that had adhered in an abnormal way and has started to pull back on its own I would think one nice tug to release that and start the healing would be much better then waiting it out.
My son's got "stuck" twice and we had to have the doctor pull it down again...hard to describe. But, we could tell he was uncomfortable because of it and he is fine now (2.5 yrs)

We all dislike going to the doctor, but this is a sensitive spot and for heavens sake he wants to feel better  Cry Cry
Plus, you will instantly get rid of the "mommy guilt" after taking him to see the doc. Wink

anynomouse.... how old was your son when you brought him in to have his skin pushed back?  I brought my little guy in before he was one and they pushed it back but not all the way.. I did it the rest of the way when I got home.... Now he's 1-1/2 yrs... and it looks like it could be pushed back a little more again.. but he won't let me touch it.. he doesn't fuss about it hurting or anything... and I wonder if i bring him back to the same doc if he will think it's okay since he didn't push it back all the way last time...  Undecided i don't know.. how annoying.

According to the American Academy of Pediatrics, to care for an intact penis, "simply wash with soap and water" and "do not forcibly retract the foreskin."   "Sometime during the first several years of your son's life, his foreskin, which covers the head of the penis, will separate from the glans."

In regards to the original question, it sounds like it either readhered and is breaking loose because of the erection OR never was unattached and is breaking loose on its own.   IMO I would continue with the AD and warm baths, give it a few days and see what happens.   If the pain is present during urinating then I would seek a doctor and/or treat for a UTI.

By the way, my oldest boy is circumsized and my youngest boy is not.

Thanks!!  I think each of you helped!  I spoke to our new pediatrician on the phone and she seemed happy that this was happening while he is young rather than during puberty.    She also said to soak it in water with Epsom salts and apply antibiotic ointment (neosporin) several times daily to ward of infection.  If it seems worse at any time or fails to show improvement after 48 hours, I'm to bring him in.  I'll post the outcome.                                              ~L

Ok, now my kid has trouble! My 20 month old came to me this morning crying saying "Poopy, mommy! My bottom!" When I took his diaper off he was just wet, but he had a red, raised, sore bump on the base of his penis, about the size of a pencil eraser. Anyone have experience with this?

Does it look like a pimple? are there more around it? It could be yeast, my son has had a yeast diaper rash before and they hurt....think of how they bug us if we get a yeast infection. Possibly Tea Tree Oil could help as it is antifungal. I have had to resort to using the antifungal cream on my son before, but it heals super fast.

KatieMac.... I remember reading something about that in one of those awful books, What to Expect..... when you are expecting or the first year... Can't remember which one.  Anyway... If you have one of those, check it out, or if someone on here has a copy they should look it up for you.. I can't remember how they said to treat it, but I just remember that it wasn't anything to be super freaked out about... It was described just like you said, "pimple-like"... Sorry this reply doesn't help much.. haha. hope you find the answer.

Hi!  At about 18-20 months old my oldest son had sorta the same thing.  His penis started to retract (already circumsised) and bled a little...it healed on its own.

Hi....I think this is the right place for this?

I was changing my 11month old little boy this morning, only to discover that the tip of his p*nis was red swollen and covered in sores. My though is possibly yeast, but I am concerned that it may be an infection...How could I tell? My baby is not circumcised, but neither is my older son and I have never seen this before. I do use cloth diapers...maybe they were harboring something?
The spots are red raises and surrounded by a white ring. I gave him a warm water bath and applied coconut oil. I would like to avoid seeing a doctor. My naturopath does not work on Sat and the ER seems to extreme. Oh and he does not seem to be in any pain even when it is touched.

Thanks for reading my scattered post......any help you have to offer would be appreciated.Thanks

I don't have any advice for you  because I am not sure of tea tree oil on the p- place Smiley  Just wanted to say I have 4 sons and a husband uncircumcised and we have  had only one incident when our oldest son has some white creamy stuff under his skin and we got some stuff from the pharmacy to put in some water and soak it in - went right away.  Other than that we have not had any problems - and we don't pull their skin - it takes care of itself eventually.  You might know that most Europeans do not circumcise - so it is not an odd thing Smiley

I would keep him clean and dry even if that means a change every 15 minutes. Let him sit and play in a bath with some sea salt in it and then let him streak if it's warm enough. At least use a hair dryer on low until the area is completely dry before you re diaper him.  I also give them extra fluids to flush out any pathogens that might be traveling up the uretha.
 I'm not one to put coconut oil on anything suspected to be fungal or "yeasty' because of the oil leaving a thin residue that blocks off oxygen and keeps it moist. I KNOW coconut oil soaks in more quickly than other oils, but still don't like to use it. Many disagree with me.  Smiley

Thanks I will keep that in mind. I only did the oiil once. When I just changed hiim it looked like it was getting better, but then I noticed a little blood in the diaper....It kinda looked like the sore had scabbed over and then stuck to the diaper causing some bleeding. At what point do you think I should be concerned enough to go to the doctor?

If the cococut oil seems to be helping, I'd keep using it. If you have, or can get, Pau d'Arco, you might want to try that too. It is a potent antifungal and antimicrobial.

The dead sea salt bath is good and I use bag balm just at the tip and that has always worked. Our midwife suggested dipping the tip in cornstarch after soaking and drying just to pull the excess moisture out that you can't reach with a towel etc. I would also give him internal things like vitamin c and garlic to boost him immune system and lots of water to flush out any nasties Wink Kim

I very gently wiped the tip and yellow goop came out. I gently wiped it out with a q-tip. I bathed him in dead sea salts. What else can I do? He is not in any pain, no fever etc. indicating infection.

This is probably not his problem, but I'd check for a hair tourniquet under the foreskin, whenever there was swelling down there. Just to put my mind at ease, before moving on to other possibilities.

My one uncricumsied son has this troublea few years ago.  It was just like you described.  His was angry red and painful and there was puss coming out when I would sightly retract the foreskin.  So I decided that I was either going to have to take him into the doctor and have him retract the forskin or circumsize him or I could retract the foreskin to see what the cause of the infection was.

It wasn't pleasant but it worked.  I retracted the foreskin to see that there was a white bead that had formed on the tip and when I forced the skin back it released the infected "bead thing" and the infection ceared up right away. 

There is a medical term for the white bead but I cannot remember the name.  My other sons have had a couple and they usually never become a problem but for some reason this one got infected.

I have 3 boys and this happens every once in awhile. Like I said earlier in the thread keeping them dry if they are still in diapers helps. Rinsing and very gently rolling the tip of the foreskin from side to side after a bath can loosen excess or dried "stuff" that should come out of there. I am just not one to forcefully separate the forskin unless absolutely necessary. White goo is normal "lubricant".
